How can I stop masturbating?

I'm 16, and every once in a while, I masturbate. I've been addicted to it for years. My church says that if I masturbate, then I will burn in hell forever. I try to stop, but I can't. I pray every day. I have read the bible 34 times now, and I have stopped hanging out with all my friend for the past 4 months, because I feel like a miserable piece of **** that is not worth anyone's time. My church leader has told me that until I can stop masturbating, I will never get into heaven. He said I can't participate in any youth activities with any of the other guys my age, or perform anything clerical until I can go at least two months without masturbating. How can I stop?
